Children who are 5 years old by September 30 are required to start
kindergarten in the fall. Registration typically kicks off in March!

You can only request a birth certificate for yourself or an immediate family member; under special circumstances, you may request a certificate for someone else.

Mail a completed Application for Certification of a Vital record to the address below with a copy of valid identification.
Cost is $12.00 and will only be accepted in the form of check or money order.

Your child must have proof of immunizations to attend school
Before your child is allowed to attend school, you must provide proof of all required immunizations. It is a good idea to complete your child’s immunizations well before the first day of kindergarten. Contact your child’s physician and ask for his/her records today.

Call 1-855-242-8282 or visit www.famis.org to enroll in Family Access to Medical Insurance Security (FAMIS), a program that helps families provide health insurance for their children.
